Spinal discs act as cushions between the vertebrae of the spine. These discs help absorb shock, support movement, and maintain flexibility within the spine.
A herniated disc occurs when the soft inner portion of a spinal disc pushes through a weakened area of the outer disc layer. When this happens, the disc material may irritate or compress nearby nerves, leading to pain, numbness, or weakness.
Herniated discs can occur anywhere in the spine but most commonly affect the cervical spine (neck) and lumbar spine (lower back).
Understanding Disc Anatomy
Each spinal disc contains two primary components:
Annulus Fibrosus
The tough outer layer of the disc that provides structural support and helps contain the inner portion of the disc.
Nucleus Pulposus
The soft, gel-like center of the disc that helps absorb pressure and allows the spine to move smoothly.
When the outer annulus weakens or tears, the inner nucleus pulposus may bulge or herniate outward.
Types of Disc Herniation
Disc herniations are often described based on the extent of disc displacement.
Disc Bulge / Protrusion
The outer portion of the disc remains intact, but the disc begins to bulge outward. This may place pressure on nearby nerves.
Disc Prolapse
The inner disc material begins to push outward but remains contained within the outer layer of the disc.
Disc Extrusion
The inner nucleus pulposus breaks through the outer annulus fibrosus but remains connected to the disc.
Disc Sequestration
A fragment of disc material breaks free and may migrate within the spinal canal.
Symptoms of a Herniated Disc
Symptoms depend on the location of the herniated disc and whether nearby nerves are compressed.
Common symptoms include:
- Neck or lower back pain
- Pain that radiates into the arms or legs
- Numbness or tingling in the extremities
- Muscle weakness
- Burning or electric-like nerve pain
When a herniated disc compresses a nerve root, it may lead to radiculopathy, which causes pain that radiates away from the spine.

Cervical Disc Herniation
A herniated disc in the neck may cause pain that radiates into the shoulders, arms, or hands.
Thoracic Disc Herniation
Thoracic disc herniations occur in the middle of the spine and are relatively uncommon. Symptoms may include pain that radiates around the chest or torso.
Lumbar Disc Herniation
Herniated discs in the lower spine can compress nerves that travel into the legs. This may cause sciatica, which produces pain that radiates down the buttock and leg.
Diagnosis and Treatment
Diagnosis typically begins with a physical examination and review of the patient’s medical history. Imaging studies such as MRI scans, CT scans, or X-rays may be used to evaluate the spine and identify disc herniation or nerve compression.
Most patients improve with non-surgical treatments, including:
- Physical therapy
- Anti-inflammatory medications
- Activity modification
- Targeted spinal injections
If symptoms persist or significant nerve compression is present, surgical treatment may be recommended to relieve pressure on the affected nerve.
